Our Front Office Staff will email you a Welcome Letter and the New Patient Forms to complete prior to your first appointment. If you have not received this yet, please contact our front office staff at: cliptak@drsofpt.com or call (949) 582-2555.

Don't forget to bring a copy of your Drivers License, Insurance Card, and PT Referral with you to your first appointment!
